The following items may be needed to complete your application for the Best Beginnings Child Care Scholarship. You may wish to gather this information prior to beginning your application. However, you will be able to save your application and return to it later. Additonal forms may be found on the ECSB website.

Identity:

• Social Security Number (Optional)

Income and Resources:

• Pay stubs, earnings statements from employers
• Self employment financial records; tax forms, business license, bank statements
• Amount received from Social Security, Unemployment Insurance benefits, Workers' Compensation, pensions, etc.
• Amount of child support and/or alimony received
• Amount received from income producing properties

Expenses:

• Self-employment expenses
• Child support to a non-household member

ePass Montana:

• An ePass account is required to access the service
• You will need to create your own personalized username and password
• IMPORTANT - Record this information. You will not be able to return to your application without it.
• If you cannot remember your username you will need to create a new ePass account and will have to start a new application.
